Lifton describes his initial distress upon hearing his informants' descriptions of what they had lived through, and how it became necessary for him to establish a psychic distance between himself and his subjects in order to work. He writes of the survivors' immediate reactions to the death and destruction around them, and recounts their lifelong concern with death as they watch for signs of radiation sickness in themselves and signs of abnormalities in their children. The account ends with the thoughts of one survivor who stresses that the atomic bomb has the power to make everything into nothing. UI - 20080 AU - Llewellyn-Thomas, H.A. ; Thiel, E.C. ; Clark, R.M. Tversky, Amos TI - On the elicitation of preferences for alternative therapies - SO - New England Journal of Medicine. 1982 May 27; 306(21): 1259-1262. AB - A large number of subjects (patients, physicians, and graduate students) were asked to imagine that they had lung cancer and to choose between two therapies based on descriptions of possible consequences. Four variables were investigated, the more important being whether or not the treatments, surgery or radiation, were identified and whether the outcomes were framed in terms of mortality or survival. While surgery was preferred by all three populations, decision making was influenced by pre-existing ideas about radiation; biases resulting from skewed, though accurate, statistics; and the presentation and nature of the therapeutic data. These findings have implications for physician-patient communication, explanations of treatment alternatives, and patient participation in medical decisions. Other issues include the organization of data monitoring committees and the problems of premature publication and exaggerated estimation in trials that stop early.
