TO:  Flight Surgeon Concerned.

	The above named Officer/enlisted man has received
Irradiation treatments for aero-otitis at this clinic as
indicated.  He should receive his next treatment 25 days
following the last treatment received.

	1st Treatment 10 August 1945 50 mg, 2 app, 8 1/2 min.

	2nd Treatment 31 August 1945 50 mg, 2 app, 8 1/2 min.

	3rd Treatment 26 September 1945 50 mg, 2 app, 8 1/2
	min.

	Request that WD AGO Form 4206 be checked, and proper
entry of these treatments be made.

	Every effort should be made to have this man receive
the [not readable] to series of three (3) treatments.
